NADRA CARD SURGER

Location:                        15 Harwoods Road, Watford, Hertfordshire, WD18 7RB

                 Date & time:                   Saturday 20th and Sunday 21 March 2010 from 9.00am-1.00pm

The surgery is for FIRST TIME applicants only. Applications for renewals/replacements/ amendments WILL NOT be processed.

Applicants will be served on an appointment basis only. Please contact Watford Muslim Community Project on 01923 223466 for a scheduled slot.

REQUIREMENTS

No photographs or pre filled applications forms are required.

Applicants whose parents have NICOP cards:

• Full birth certificate stating the names of the parents
• Valid British or Pakistani passport (newborns may use fathers passport)
• At least one parents NICOP/CNIC card

Applicants with NIC:

• NIC card
• Valid British or Pakistani passport
• Either parents NICOP card or brothers/sisters/real uncle/aunts NICOP/CNIC card

Applicants with passports only:

• Valid British or Pakistani passport
• Birth certificate if do not have Pakistani passport
• Either parents NICOP card OR brothers/sisters/real uncle/aunts NICOP/CNIC card

Where applicants are unable to provide actual cards then photocopies of the card or the card number will be acceptable.

The fee is £36.00 for applicants as set by the Pakistan High Commission and is payable in CASH only.
